Transition Assistance

We offer social services to assist patients with a successful transition from pediatric to adult care. Our team provides personalized support and guidance to ensure a smooth transition process, empowering patients to take control of their health journey with confidence.

Transportation Assistance

We understand that getting to medical appointments can be a challenge for many patients. That’s why we offer free transportation assistance for patients in need of transportation to medical appointments related to their sickle cell disease. Our goal is to ensure that every patient has access to the care they need, regardless of transportation barriers.

Counseling and Support Services

Living with sickle cell disease can be emotionally challenging, and we’re here to provide support every step of the way. We offer genetic trait testing and counseling to help individuals understand their risk of sickle cell disease and make informed decisions about their health.
